    Responsibilities

    The Triage Nurse will be responsible for, but is not limited to:

     

    1. Answer incoming calls from patients and families to assess the needs of the patient to determine the appropriate care needed

    2. Document relevant clinical information within the electronic medical records system (Suncoast)

    3. Documentation of patient care, needs and progress

    4. Educate and instruct Aides, caregivers and family members on patient's care

    5. Attend team conferences 6. Administer skilled nursing care to patients on an intermittent basis in the home

    Qualifications

    Education:

    Associate's Degree in Nursing, Bachelor's preferred

     

    Professional Experience:

    • Must have a State Certified RN license
    • 2 years' experience as a professional nurse, hospice experience preferred
    • Must have a valid Driver's license and reliable transportation
    • Must be willing to travel
    • Strong verbal and written communication skills
