    The Continuous Care LPN will be responsible for providing direct patient care, health counseling and teaching to patients and their families. Responsibilities can include, but are not limited to:

    1. Reinforcing teaching, supervising and counseling of the patient, family members and staff regarding nursing care and the patient's needs.
    2. Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis
    3. Delivers physical and psychological care as prescribed by the attending physician, as supervised by the Hospice Nurse and as approved by the IDC team
    4. Maintains continuity of patient care through ongoing communication with families and other health care professionals
    5. Provides direct skilled bedside nursing care.
    6. Evaluates patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
    7. Evaluates effectiveness of treatments and monitors patient's reactions
    8. Maintains accurate clinical notes on interdisciplinary charts.
    9. Assists in coordination of hospice services, appropriately reporting the identified needs for other disciplines to the RN Case Manager and participating in Interdisciplinary Team meetings



    High School Diploma or equivalent


    Professional Experience:

    • Must have a valid LPN license for the state of operations
    • Must have 2 years' experience, preferably within hospice
    • Proficient knowledge of Electronic Medical Records systems, a plus
    • Proficient in Microsoft Office, Excel, & Outlook
