    Hours: Monday through Friday, 8:30am to 5pm

    The Bereavement Coordinator, will be responsible for but not limited to:

    1. Maintain and file all Bereavement paperwork according to Ascend Hospice Protocol
    2. Ensures compliance with care plan contacts for bereaved according to grief risk and contact frequency
    3. Maintain active list of State Support groups
    4. Prepares mailing labels and letters for bereavement mailings to bereaved family and/or friends in the scheduled time frame (5 months, 12 month anniversary cards, and 13 month bereavement letters to bereaved)
    5. Refers family and fiend to assigned chaplain for further assistance
    6. Follows verbal and written instructions accurately and in a timely manner
    7. Creates bereavement reports from HPMS system
    8. Audits to maintain compliance
    9. Places 2-3 month follow up calls to low risk bereaved
    10. Completes documentation of bereavement calls according to compliance guidelines
    11. Performs copying and routine coping as requested
    12. Distributes mail and faxes to appropriate personnel
    13. Sends outgoing mail and faxes as requested
    14. Keeps files, records and supplies well organized
    15. Follows verbal and written instructions



    BA or Masters Degree Preferred


    Professional Experience:

    • 2-4 years of related experience
    • Strong Organizational Skills
    • Clerical/Administrative experience
