    Responsibilities

    The Intake Manager is responsible for, but not limited to:

    1. Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator
    2. Insures that whenever possible, all referrals are converted to admissions
    3. If referrals do not convert to admissions, the Administrator will be notified
    4. Track all inquiries and referrals daily. This report will be given to the Administrator weekly
    5. Assists patients, families and responsible parties in completing the admissions packet as needed
    6. Works collaboratively with appropriate staff to insure written or verbal consents are obtained
    7. Will collaborate with Administrator or designee before referrals are not converted to admissions
    8. Reviews referrals with hospice physician (s) to explore admission diagnosis and clinical history to support medical eligibility.
    9. Takes hospice physician orders as needed for admissions
    10. Collaborates with Billing to ensure financial status is determined and or authorized is obtained
    11. Monitors and collaborates with medical records to ensure the census is reported correctly

    Qualifications

    Education:

    Associates Degree in a related field

     

    Professional Experience:

    • Licensed as a Registered Nurse (RN) within the state of Operations
    • CHPN Certification, a plus
    • Preferably 5-8 years
    • Experience in a related capacity
    • Must have a Valid Driver's License and reliable source of transportation
    • Must be willing to travel
    • Excellent communication, interpersonal and organizational skills
